If you wish to report any feedback or a complaint, please complete the Physiotas Feedback & Complaints form here.
You can also download the form here or you can obtain a hard copy at any of our practices.
Completed forms can be dropped into your local practice and placed in the Feedback & Complaints box at reception, or handed to one of our staff to pass on to Management.
You can also express any concerns directly to the NDIS here.
